NYSHIP Family coverage includes coverage for your children.
The following are eligible for NYSHIP coverage until age 26:
Under NYSHIP definitions, you may cover "other" children if they meet these requirements before age 19:
To enroll an other child under your NYSHIP coverage, you must complete Enrollment or Recertification of an “Other” Child as a NYSHIP Dependent Form (PS-457). File the completed form with your HBA or Agency Contact and verify eligibility with supporting documents (listed in the form). You will need to do this upon initial enrollment and every two years thereafter.
You must contact your HBA or Agency Contact to complete the appropriate forms to add your newborn child to your coverage. Your newborn child is not automatically covered without these forms.
Add your newborn (natural or adopted) within 30 days of birth to avoid potential delays and/or fees. If needed, you can switch from Individual coverage to Family coverage during this time.
